Does MyPlan offer takeover benefits?If you were previously covered under a dental plan, you may be eligible for takeover benefits, which means waiting periodsare waived.

You will be asked to provide an evidence of coverageletter from your prior carrier, which must include a terminationdate of the prior plan that is no more than 30 days prior to thedate we receive your application for MyPlan coverage.

Who is eligible to purchase MyPlan?MyPlan insurance coverage is available in states where it’s approved to anyone age 18 and older who does not havecoverage through another Ameritas dental plan. You can request coverage for your dependents; dependent eligibilityvaries based on state law.

Can I see the dentist I have now?Yes, you are always free to visit the dentist of your choice. MyPlan members also have more than 130,000 Ameritas PPOprovider access points nationwide for dental care, which meansyou benefit from credentialed dentists who offer a discount onservices provided. When will my policy be effective?Your policy will be effective on the first day of the month following the approval of your application and collection of your first month’s premium.

Are my rates guaranteed?Your rates are guaranteed for 12 months following your plan’s effective date. After that, you will receive at least 30 days’ notice (more if required by state law) if your rates change.

Are there services that are not covered?Yes, some services such as orthodontia are not covered. Procedures begun prior to your effective date are not covered. MyPlan does not provide benefits for lost or stolen appliancesor cosmetic procedures. It does not cover hospitalization orprescription drugs. Certain covered expenses may be subject toa waiting period (an elimination period). At times, two or moreprocedures are considered adequate and appropriate treatment.In this case, the benefit will be based on the charge for the least expensive procedure.This is not a complete list of exclusions. Your policy will contain a complete listing of exclusions, procedures covered and any frequency or other limitations on specific procedures.

Do I have coverage outside of the state I live in?Yes, if you are traveling or have a covered student living in a different state, you will still have coverage.

What if I want to cancel the policy?All cancellations must be submitted in writing to Greater Insurance Service Corp., attn. Central Billing Service, P.O. Box8633, Madison, WI, 53708-8633. Once the written request isreceived, the policy will be cancelled the later of the first day of the following month or the requested cancel date (must be the first of a month).

Tips for dealing with Dental Emergencies

Accidents do happen, and knowing what to do whenone occurs can mean the difference between savingand losing a tooth.

Most dentists reserve time in their daily schedules foremergency patients. Call your dentist and provide asmuch detail as possible about your condition.

Clean the area gently with a cloth and apply a cold compressto reduce any swelling. If the bleeding doesn’t stop, go to ahospital emergency room immediately.

Rinse your mouth with warm water to clean the area. Use acold compress on the area to keep any swelling down. Callyour dentist immediately.

jaw possibly broken

Apply a cold compress to control swelling. Go to yourdentist or a hospital emergency room immediately.

knocked out tooth

Hold the tooth by the crown and rinse off the root of thetooth in water if it’s dirty. Do not scrub it or remove anyattached tissue fragments. If possible, gently insert and hold the tooth in its socket. If that isn’t possible, put the tooth ina cup of milk and get to the dentist as quickly as possible.Remember to take the tooth with you!

Try to gently remove the object with dental floss; avoidcutting the gums. Never use a sharp instrument to removeany object that is stuck between your teeth. If you can’tdislodge the object using dental floss, contact your dentist.

Rinse your mouth with warm water to clean it out. Gentlyuse dental floss or an interdental cleaner to ensure that thereis no food or other debris caught between the teeth. Neverput aspirin or any other painkiller against the gums near theaching tooth because it may burn the gum tissue. If the painpersists, contact your dentist.
